
Rehab Without Walls does exactly what its name suggests, offering rehabilitation services meeting clients wherever they are instead of a traditional facility setting.

“Our services are designed around each individual’s unique needs and goals and may include rehabilitation therapy, nursing, behavioral health support, personal care, case management and other specialized services,” Patel added. “We work closely with individuals, families, caregivers and other providers to help people build skills, increase independence and participate more fully in their communities.”

While on the surface, Rehab Without Walls may sound similar to home health services, there are some key differences, according to the Rehab Without Walls website. Rehab Without Walls provides more focused and outcome driven rehabilitation therapies than home health services. Additionally, home health cannot continue to treat patients once they leave the house. Home health is also a medical heavy model with more emphasis on nursing as opposed to rehab.
The Rehab Without Walls model combines science, compassion and creativity to deliver the best results for each individual. They start with an assessment to determine a person’s current level of functioning in three categories including activities, impairments and participation. The assessment is followed by goal setting with the individual and family then assembling an interdisciplinary team for support – such as physical therapists, vocational counselors and neuropsychologists – before creating and implementing a personalized rehab plan.
