Axis Community Health Receives Top Marks for Patient Care

The mission of Axis Community Health is to provide quality, affordable, and compassionate health care services that promote the well-being of all members of the community. In August, officials announced that Axis has been named one of OCHIN Health Center Controlled Network’s (HCCN) Top Performers for the 2022 to 2025 program cycle. OCHIN is a national nonprofit health care consultancy that helps participating health centers optimize their use of health IT. The Top Performer designation is HCCN’s highest distinction, recognizing exceptional healthcare delivery and community impact.

The prestigious recognition celebrates Axis Community Health’s commitment to delivering comprehensive, whole-person care to its diverse patient population. The OCHIN HCCN is a collaborative group of health centers addressing operational and clinical challenges related to health IT. It includes dedicated health IT consulting tailored to support unique organizational needs and focused cohorts to foster peer-to-peer sharing, collaboration, and problem-solving, and the support of the industry standard patient health record system, EPIC. Axis has been working with OCHIN since 2019.

In addition to the OCHIN Top Performer designation, Axis earned two significant acknowledgements from the Health Resources and Services Administration (HRSA), a federal agency of the U.S. Department of Health and Human Services. The first, Improving Health Care Access Recognition, was the result of Axis Community Health demonstrating “measurable growth in community service by increasing total patient count and expanding access to essential services including mental health, substance use disorder, vision, dental, and enabling services by at least 5% year-over-year,” according to Axis officials. The second recognition from HRSA, Advancing Health Information Technology for Quality Badge, “highlights the organization’s innovative use of technology to enhance patient care through telehealth services, patient engagement via health IT platforms, clinical information exchange with healthcare partners, and comprehensive collection of social determinants of health data.”

“We’re incredibly proud of these recognitions,” according to Angelina Speltz, Senior Director of Quality Enhancement at Axis Community Health. “It reflects our team’s dedication to excellence and validates our approach to comprehensive patient care. This achievement motivates us to continue setting the standard for quality healthcare in our community.”

On August 21, Axis Community Health officials held a ribbon-cutting ceremony for its newest clinic. The clinic, located at 1686 Second Street in Livermore, includes dental and medical examination rooms as well as offices for behavioral and social support services. Founded in 1972, Axis Community Health is a Federally Qualified Health Center dedicated to providing quality, affordable, and accessible healthcare services to the Tri-Valley communities of Dublin, Pleasanton, and Livermore.

“We are now well on our way to providing more than twenty-thousand visits each year to five thousand six-hundred patients, offering eight medical exam rooms, six dental operatories, behavioral health offices, and social support services–all under one roof,” noted Chief Executive Officer Liz Perez-Howe at the ceremony. “That’s not just numbers; that’s thousands of stories of quality health care happening right here.”

Perez-HoweShe added, “This building is more than bricks and mortar. It is a promise. A promise that when someone needs help, we will be here. A promise that healthcare will be delivered with dignity, compassion, and excellence. And it’s also a symbol of what happens when cities, donors, staff, and neighbors come together to say, ‘We will not accept barriers to care. We will make a way.’”
